Admission Requirements

• Students are accepted on a full-time basis
• Parents attend a 30 minute orientation with the center’s director to review school policies
• Completion of Registration Forms to include:
  
  1. Registration Form
  2. Family Questionnaire
  3. Immunization Form
  4. Emergency Medical Consent Form
  5. Permission Slip for School Activities within Walking Distance of the Center Neighborhood
  6. Child Care Services Contract Specifying Tuition Rate
  7. Accounting Policy Acknowledgement Form
  8. Electronic Funds Transfer or Credit / Debit Card Payment Authorization Forms
